The form must be brought to Mr. Webber printed two-sided.  IT MAY NOT BE TWO PAGES STAPLED TOGETHER!

It must be filled out completely or it will not be accepted.  A current and active parent/guardian phone number must be included.

At the top of page 1, fill in the following information:
